They're certainly some mysterious quirks. I tend to leave Claustrophobia on my heroes, and I don't think I've seen it trigger anything before. I know it can cause claustrophobia on a bad outcome, and that it can be triggered by Bloodthirsty and I think Necromania and Demonomania (though I could be wrong on either of those). Talk about rare!
